Boarding/Daycare Contract

This is a contract between Green Acres K-9 Resort and the pet owner whose signature appears below (here-inafter called “Owner”).

I choose to let my dog participate in the daycare program of Green Acres K-9 Resort which includes, but is not limited to, having my dogs play with other dogs of different breeds and size, playing on a variety of dif- ferent surfaces that other dogs have played on and being out in the 10 acres of field off-leash.

I understand the extra risks involved in letting dogs play together as far as disease transmission, risk of injury and risk of escape.

I have read, agree to and understand the entire Boarding/Daycare Contract Terms
